ご来院Visit the Clinic
新橋駅日比谷口から徒歩2分。ダイワロイネットホテル新橋の地下1階です。2-minute walk from Shinbashi Station (Hibiya Exit). We are located on the B1F of Daiwa Roynet Hotel Shinbashi.
受付・問診Reception & Medical Questionnaire
受付にてお名前をお伝えください。問診票にご記入いただきます。Please give your name at reception. You will be asked to fill in a medical questionnaire.
事前にWEB問診票をご入力いただくと、スムーズにご案内が可能です。該当する診療科目の問診票をご入力ください。Completing the online questionnaire in advance will allow for a smoother visit. Please fill in the form for the relevant service.
診察Consultation
医師が丁寧にお話をお伺いし、診察・検査を行います。The doctor will listen carefully to your concerns and conduct an examination and tests as needed.
会計・お薬Payment & Prescription
診察後、会計をお済ませください。必要に応じてお薬を処方します。After your consultation, please proceed to payment. Prescriptions will be provided as needed.

- 健康保険証Health Insurance Card
- お薬手帳（お持ちの方）Medication Record Booklet (if you have one)
- 紹介状（お持ちの方）Referral Letter (if you have one)
- 健康診断の結果（お持ちの方）Recent Health Checkup Results (if available)

自由診療（更年期・ホルモンケア等）をご希望の方へFor Those Interested in Private Medicine
自由診療は、性差によるお悩みや加齢に伴う変化、コンディション管理など、保険診療の枠を超えたケアを提供します。料金については診察前に丁寧にご説明します。Private medicine covers care beyond public insurance — including gender-related concerns, age-related changes, and condition management. Fees will be explained in detail before your consultation.
